Supportive Apartments
The Supportive Apartment Program is a possible next step toward independent living for individuals transitioning out of a community residence or those who are not suited for community residence life. There are six supportive apartments certified by the NYS Office of Mental Health located throughout the City of Cortland. This level of community living is appropriate for adults who do not need the constant support and supervision of a community residence but do require continued treatment, frequent monitoring, help with achieving goals and daily living skills development.
Qualifications:
Program participants are adults (18 years or older) diagnosed as seriously and persistently mentally ill (SPMI) who are motivated to move toward and capable of independent living. In addition, the person must be limited in their ability to successfully function in a more independent setting. Admission requires a referral from a clinical professional and review and approval by the single point of access (SPOA) committee. Participants must be able to live in an apartment setting with a limited possibility of serious harm to themselves or others and possess the potential to improve their daily living skills.
Payment:
Individuals participating in the Supportive Apartment Program are responsible for a monthly program fee that is paid in a variety of ways: through Temporary Assistance, NYS Medicaid, Social Security benefits, or private income. Although admission is not based on ability to pay, residents are expected to pursue the entitlements available in order to pay for their residential care and treatment. A sliding fee scale is available for people who pay privately.
Participation:
The resident works closely with a Residential Counselor to develop a plan outlining his or her individual goals. The individual and counselor meet frequently to discuss progress or concerns as they relate to recovery, mental health and daily living. The counselors assist with transportation, budgeting, medication management and symptom management and can be reached 24 hours a day in case of emergency.
Typical Length of Stay:
Similar to other programs the length of stay varies depending on the needs of the individual. An average participation is from six months to a year.
Program Expectations:
Participants must agree to meet frequently with the Residential Counselor and work on personal goals that will advance his or recovery and move toward independent living. The resident must understand their medication regime and make effort to comply with treatment, maintain the upkeep and management of their apartment, abide by landlord rules, attend all necessary appointments, and follow agency procedures for reporting guests, overnight trips and extended absences.
